Mission Rebuild HUD is a new program that will house 15 homeless and disabled veterans and families of veterans in permanent supportive housing. The case manager will assist qualified veterans and veteran families with transitioning from homelessness to permanent housing, and provide intensive weekly case management to housed clients. Requires conducting housing searches with clients, working with landlords to place clients, and providing resources and services that enable the client to maintain housing and increase independence. The case manager also participates in collecting documentation for intake, processing of rent checks, inspections, and re-certifications of eligibility for HUD-assisted housing for his/her clients.
