Case Manager ( Families)
The Family Housing HUD program provides permanent supportive housing to 14 homeless/formerly homeless client families with a disabled adult. The case manager provides intensive weekly case management in HUD-subsidized scattered site supportive housing. Requires conducting housing searches with clients, working with landlords to place clients, and providing resources and services that enable the client to maintain housing and increase independence. The case manager also participates in collecting documentation for intake, processing of rent checks, inspections, and re-certifications of eligibility for HUD-assisted housing for his/her clients.
