Full time (31 hours per week)

Provides case management/brokerage services to Behavioral Health Services clients to access medical, educational, social, prevocational, vocational, housing, rehabilitative or other needed community services for eligible individuals.

Special assignments include coordination with social services, treatment providers and probation officers, assistance to judges in the courts, support to the individual clients to comply with various program requirements including grant projects. May provide supportive counseling activities as part of the case management role with individual clients.
