P/T Case Manager

GENERAL NATURE OF POSITION: Provides integrated case management support and skills training for mental health and alcohol and drug services to assist homeless veteran clients to seek housing, and to supportive housing residents with severe mental illness and other physical disability; to access medical, educational, social, prevocational, vocational, rehabilitative or other needed community services for eligible chronically homeless, disabled veterans and families. Conducts outreach to the homeless veteran population and provides case management to link clients to veterans’ and other community services.
